Required Submissions
1. Photo of Passport
• Please upload this under the submission section: I-9 Identification List A
• Your passport photo and number must be clear and readable
• Your passport CANNOT be expired
2. Form I-94
• Arrival/Departure Record, indicating F-1 nonimmigrant status
• Please upload this under the submission section: I-9 Identification List B
3. DS-20 (Your CPT paperwork)
• This is a form issued by your university that authorizes the CPT employment
• Please upload this under the submission section: I-9 Identification List C
